What to bring to exam?

ID

  • A government-issued photo identification document (passport/green card/driver's license), document containing your alien/USCIS number

Vaccines

  • If available, please bring any copies of domestic or foreign immunization records. If you don't have it, NO PROBLEM. We will provide you with a blood test order to determine your immunization from blood work analysis

Tuberculosis

  • Testing for Tuberculosis - All applicants 2 years old or older must have an interferon gamma release assay (IGRA)- Blood. If positive must have a chest radiograph (chest x-ray)

Syphilis

  • Testing for Syphilis*** age dependent

Gonorrhea

  • Testing for Gonorrhea *** age dependent

No immunization records? 

  • Laboratory evidence of immunity is acceptable for measles, mumps, rubella, hepatitis A, hepatitis B, and varicella if the applicant lacks acceptable documented history of vaccination for these diseases. We will order titers - blood test to  check your immunity for those infections

  • If your titers comes back that you do not have any immunity, meaning you never were sick or were vaccinated, we will refer you to the department of health to complete your vaccinations or to any Pharmacy.

Effective April 4, 2024, a Form I-693 that was signed by a civil surgeon on or after Nov. 1, 2023, does not expire, and can be used indefinitely 

Final Report

  • You will review your report 5-7 days

  • You you will get a copy of the report

  • And you will get officially sealed report

You will come for your final report after we will get your blood work results
